Client Agreement
I understand and agree the following:
- Prior to commencing my treatment, I will attend a local doctor for an initial general examination and diagnosis and send the report to Bruce Du Ve, The Merrion Clinic, 50 Merrion Square East, Dublin 2, Ireland.
- I will regularly attend my local doctor at my discretion to have my health condition actively monitored during the time I am receiving input from Bruce Du Ve.
- I will commence any program of dietary modification, cleansing or supplementation after notifying my doctor.
- I confirm that I will keep Bruce Du Ve advised of any changes to my medical circumstances, which may arise subsequent to this date and that I am wholly responsible for the consequences of any failure to do so.
- I will not modify my regimen without notifying my doctor and Bruce Du Ve, except in an emergency situation, in which case I shall immediately notify both parties.
- I will contact my doctor immediately in the case of any emergency or concern I may have about my health condition and immediately pass on my doctor’s report to Bruce Du Ve.
- I will take any tests requested by Bruce Du VE and have the results forwarded to the clinic and my doctor.
- Should I believe I have a particular health issue, I will refer myself to the relevant medical professional in order to gain a diagnosis. It may, therefore, be necessary to delay treatment with Bruce Du Ve until a diagnosis has been confirmed.
I hereby confirm that I fully understand and accept the contents of the Client Agreement.
